All entries should include the following:

• Name
• Address (including city, state, zip)
• Phone Number
• E-mail Address (if any)
• Title
• Submission Type (fiction, poetry, drawing, graphic design, etc.)

Entrants: Files submitted through this online form need to be 2MB or less. Larger files can be submitted on a CD to the Images office (X-157). If an entry has multiple files (like source files), please submit on CD. Please complete the entire form before submitting. If you have any questions/concerns please contact Images immediately.
